In order to schedule an appointment, please call our front office at 602-265-9900. We make every effort to schedule you so that your wait time is minimal; however, emergencies and other unforseen events sometimes cause delays. We will do our best to keep you informed. In case of a life threatening emergency, please call 911 immediately!
If you are unable to keep an appointment and would like to cancel or reschedule, please contact the office at least 24 hours in advance.
